Food for foreigners in Thailand: How to order when you do not speak Thai

When I was living in China, it was very difficult to order at restaurants.
So, friends of mine and myself used to carry books, dictionaries, but that was too much and complex.
I found an easy way to help people not speaking the language to order food.
Recently, we have an intern lawyer from Canada and she didn't speak a work of Thai. We prepared a document like this one:

It is written the name in THAI, also the pronunciation in Thai, so she can practice, learn the language and the meaning in English.
I used similar tricks in China, or other countries and it works well.
So, if you are a tourist or a new person in Thailand, you select the dishes you think you like, you add some drinks, like BEER or words like NO ICE or NOT SPICY and ask to translate.
You bring your sheet of paper anywhere, put it in your wallet, and I am convinced that you can order something almost every where without speaking a word of Thai.
That works well for our intern. And now, she starts to know the dishes in Thai!
